Photograph Authorization
By filling out the information below, you agree to authorize STMI Church to publish photographs taken of yourself and/or the minor child(ren) listed below, and your names and likenesses, for use in Church’s online and video-based marketing materials, and other STMI Church publications. Further, you attest that you are the parent or legal guardian of the child(ren) listed below and that you have full authority to consent and authorize STMI to use their likeness and names. You further acknowledge that participation is voluntary and that neither you nor the minor child(ren) will receive financial compensation of any type associated with the taking or publication of these photographs or participation in STMI marketing materials. You, hereby, release the STMI, its contractors, its employees and any third parties involved in the creation of STMI publications, from liability for any claims by you or any third party in connection with your participation or the participation of the minor child(ren) listed below.
